Ever have a need for evaluation at the fields (any HFC event in
the Huntsville area – practice, game, etc). HFC has a Huntsville
Hospital Athletic trainer on call at all times. They will come
to the field and do an evaluation before going to a doctor.

State League Game Change/Reschedule Request
State Fee Schedule
These
are the fees and timelines for all state documentation.
Guest player Form
This
form is to be used any time you want to bring along a guest player for a
state game or a tournament. Fees: $5.00 per player if paid
more than 7 days prior to the tournament After that the fee is $25.00
per player. ALL GUEST FORMS MUST BE SUBMITTED BY WEDNESDAY
the week of the tournament.
Must be typed!
Tournament Roster
To be used
when taking multiple guest players to a tournament. This should be
submitted to the state office at least 2 weeks prior to the tournament.
No rosters will be processed after the Wed the week of the tournament.
Must be typed!
Intent to Travel
To be used when traveling out of
the state of Alabama There is no fee.
